TAYLORSVILLE – Our beautiful and beloved daughter Mellisa Ann Worthen passed away on February 22, 2016. She was born March 30, 1977.

 She is survived by her son, Bryon Braxton Worthen, who she loved more than anything; father,  Bryon Worthen; mother, Teresa Dukes Schultz; and stepmother, Jane Robinson; brothers, Ron (Ashley) Worthen  and Adam Dukes; her stepbrothers and sisters and their families; grandmother, Nancy Fife; grandfather, Frank Medina (California); two special nephews, Guage Sean Dukes and Presley James Dukes; loving aunts, uncles, and cousins.

She was preceded in death by her partner, Misty K. Parish. She was a kind, compassionate, funny and fun loving soul. She will be missed so much.

Graveside Farewell will be held Friday, February 26, 2016, at 3 pm at Lindquist’s Washington Heights Memorial Park, 4500 Washington Blvd, Ogden, Utah.
